The Forever Active Kent Fund is being administered by Active Kent & Medway on behalf of Kent County Council (KCC).

Before filling in this form, please ensure that you have fully read the Terms & Conditions and ANNEX 1 documents sent with your funding award email.

You will need to upload several documents that were listed in that email, so please have them to hand before starting to complete this form, as you will not be able to save and continue at a later date.

You are required to download the DPIA Data Collection Form via the link below, and upload the completed form in the Policy / Document Upload section of this form.
